About the Survey
The California Healthy Kids Survey (CHKS)
2009-2010 School Year
Newark’s 5th, 7th, 9th and 11th graders are being asked to take the California Healthy Kids Survey, sponsored by the California Department of Education. This is a very important survey that will help promote better health among our youth and combat problems such as drug abuse and violence. Students do not have to take the survey. Participation is voluntary. Parental permission is required for 5th graders to take the survey. Parents of all students who are asked to take the survey are notified by mail and may request that their child not participate in the survey.
Survey Content. The survey gathers information on behaviors such as environmental and individual strengths and assets; alcohol, tobacco, and other drug use and bullying and violence.You may examine the questionnaire in the school office or select the appropriate link below.
It is Voluntary. Students who agree to participate (with parental permission) only answer the questions they want to answer and may stop taking the survey at any time.
It is Anonymous. No names are recorded or attached to the survey forms or data. The results will be made available for analysis only under strict confidentiality controls.
Administration. The survey will be administered during the first two weeks in December. Each school has scheduled a time most convenient for their site. It will take about one class period to complete (about 50 minutes) and will be administered in the students’ regular classrooms.
