Documentation/Requirement List for New (incoming) Students

Kindergarten

1. Birth Certificate of Live Birth from the county of the birth place
2. Proof of Residency (mortgage statement or rental agreement and PG&E bill)
3.  Immunization Records and/or proof of an appointment from the doctor
4. Proof of Physical Examination from the doctor or proof of an appointment
5.  Dental Screening
6. School Registration Packet from the school office

First Grade

1. Same as above

Second through Twelfth Grades

1. Proof of Residency
2. Transfer paperwork from school (this will include proof of age and immunizations)
